Symptom: thrashing, staying BSY and no ID
Cause: damaged firmware, bad sectors and dead head 4
Solution: disabled media cache, changed heads

Cause: corrupt glist, head 0 has read issues, bad sectors
Solution: cleared glist

Terminal Error: DTC Init (or) Training Failed
Fix: Saved ROM and wrote to donor PCB

Cause: Weak/Crashing Heads
Solution: Mirrored 93%, mostly with head 2 off in the latter part of the drive
